Anterior intercavernous sinus

The anterior intercavernous sinus is a small but important venous structure within the cranial cavity, forming part of the extensive venous plexus surrounding the pituitary gland. It acts as a transverse venous channel connecting the left and right cavernous sinuses, located anterior to the pituitary stalk (infundibulum) and optic chiasm. This sinus plays a role in balancing venous blood flow between the two cavernous sinuses and is considered a key structure in the circulus venosus of the pituitary gland (circular sinus).

Function

  • Facilitates venous communication between the cavernous sinuses

MRI Appearance

  • T1-weighted images:

    • Appears as a hypointense (dark) linear or curvilinear structure anterior to the pituitary stalk, between the cavernous sinuses

    • Sometimes may not be visualized unless contrast is used

  • T2-weighted images:

    • Typically hypointense or flow void, similar to other venous channels

    • May show signal void due to fast-flowing blood

  • FLAIR images:

    • Usually not well visualized unless abnormal (e.g., thrombosis or slow flow)

    • Normally, the sinus remains dark or inconspicuous on FLAIR

CT Appearance

  • Non-contrast CT:

    • Usually not well visualized due to small size and proximity to bone and soft tissue

    • May be seen as a faint linear lucency anterior to the pituitary fossa in thin sections

  • Contrast-enhanced CT (CT venography):

    • May appear as a small enhancing structure connecting both cavernous sinuses anteriorly

    • Better delineated on thin, high-resolution, contrast-enhanced images
