Anterior root of medial meniscus

The anterior root of the medial meniscus is a strong fibrous attachment anchoring the anterior horn of the medial meniscus to the tibial plateau. It plays a crucial role in maintaining meniscal stability, distributing load across the tibiofemoral joint, and preventing meniscal extrusion. Anatomical integrity of the anterior root is critical for normal biomechanics of the knee joint. Tears or avulsions of this structure can significantly impair load transmission and predispose to early osteoarthritis.

Location

  • Anchors the anterior horn of the medial meniscus to the anterior intercondylar area of the tibia

  • Positioned just anterior to the tibial attachment of the anterior cruciate ligament (ACL)

  • Lies close to the tibial spine and anterior aspect of the tibial plateau

Relations

  • Anteriorly: Tibial plateau and anterior joint capsule

  • Posteriorly: Tibial attachment of the ACL

  • Medially: Body of the medial meniscus

  • Laterally: Intercondylar eminence and lateral tibial structures

  • Superiorly: Femoral condyles (during articulation)

  • Inferiorly: Tibial plateau subchondral bone

Function

  • Provides firm anchoring of the medial meniscus to tibia

  • Maintains meniscal hoop stresses during load bearing

  • Prevents anterior translation or extrusion of the medial meniscus

  • Plays an indirect stabilizing role in knee rotation and translation mechanics

Clinical Significance

  • Tears/avulsions: May occur with ACL injuries or trauma

  • Degeneration: Common in middle-aged and elderly, often associated with osteoarthritis

  • Instability: Root tears predispose to meniscal extrusion and cartilage degeneration

  • Surgical relevance: Important in meniscal root repair and knee arthroscopy

  • Imaging relevance: Often subtle; requires high-resolution MRI or MR arthrography for detection

MRI Appearance

T1-weighted images:

  • Appears as a low-signal fibrous band attaching the anterior horn of the medial meniscus to tibia

  • Surrounded by intermediate-signal meniscus and bright intra-articular fat

T2-weighted images:

  • Normal root shows low signal intensity

  • Tears or degeneration appear as bright linear or focal hyperintensity within or adjacent to root attachment

STIR (Short Tau Inversion Recovery):

  • Root remains dark (low signal) in normal state

  • Pathological root (edema, tear) shows bright hyperintensity

Proton Density Fat-Saturated (PD FS):

  • Root attachment normally dark

  • Partial or complete root tears appear as bright hyperintensity, often with adjacent meniscal extrusion

T1 Fat-Sat Post-Contrast:

  • Normal root shows little or no enhancement

  • Inflammation, postoperative changes, or synovitis adjacent to root may show enhancement

3D T2 SPACE:

  • Root appears as a dark band-like structure with excellent contrast against bright joint fluid

  • Provides high-resolution visualization of the root attachment and tear configuration

MRI Arthrogram Appearance

  • Normal root: low-signal fibrous band surrounded by injected contrast

  • Root tear: contrast insinuates into or through the root attachment, outlining disruption or avulsion

  • Meniscal extrusion may be more evident with joint distension

CT Appearance

Non-Contrast CT:

  • Root attachment not distinctly visualized

  • Chronic avulsions may show subtle bony irregularity or enthesophytes

Post-Contrast CT (CT Arthrogram):

  • Normal root: seen as a non-opacified low-density band surrounded by contrast

  • Root tear: contrast tracks into the root attachment site, outlining a defect

  • Avulsion: contrast fills separation between meniscus root and tibial plateau
