Body of pancreas

The body of the pancreas is the elongated central portion of the gland, situated between the neck and the tail. It crosses transversely in the upper abdomen, lying retroperitoneally, and extends to the hilum of the spleen. The body is the largest portion of the pancreas and plays an important role in both exocrine (digestive enzyme production) and endocrine (hormonal secretion) functions.

Because of its proximity to major vessels and abdominal organs, the pancreatic body is a key region in radiology and surgery, especially in relation to pancreatic cancer, pancreatitis, and vascular involvement.

Location and Relations

  • Anteriorly: Stomach (separated by the lesser sac) and transverse mesocolon

  • Posteriorly: Aorta, left kidney, left adrenal gland, splenic vein, superior mesenteric vessels

  • Superiorly: Splenic artery runs along the upper border of the body

  • Inferiorly: Jejunum loops and ligament of Treitz region

  • Laterally:

    • Right: Pancreatic neck

    • Left: Pancreatic tail and splenic hilum

Function

  • Exocrine: Produces digestive enzymes (amylase, lipase, proteases) secreted into the pancreatic duct, aiding digestion in the duodenum

  • Endocrine: Contains pancreatic islets (islets of Langerhans) that secrete insulin, glucagon, and somatostatin to regulate blood sugar

  • Maintains continuity of the main pancreatic duct (duct of Wirsung) across its length

  • Provides structural support for vascular relations of splenic vessels

Clinical Significance

  • Pancreatic carcinoma: Tumors in the pancreatic body often present late and may involve splenic vessels or adjacent retroperitoneal structures

  • Pancreatitis: The body may show edema, necrosis, or pseudocyst formation

  • Surgical relevance: Important in distal pancreatectomy and splenectomy procedures

  • Vascular involvement: Splenic vein thrombosis and splenic artery encasement are common complications of pancreatic body pathology

  • Cysts and neoplasms: Serous or mucinous cystic neoplasms frequently involve the pancreatic body

MRI Appearance

T1-weighted images:

  • Normal body: homogeneous intermediate signal intensity, slightly brighter than liver

  • Fat planes around the gland: bright, defining glandular margins

  • Tumors: hypointense compared to normal pancreas

T2-weighted images:

  • Normal body: low-to-intermediate signal intensity

  • Pancreatic duct: bright linear tubular signal running within body

  • Cysts/collections: bright hyperintensity; tumors often mildly brighter than normal tissue

STIR (Short Tau Inversion Recovery):

  • Normal body: low-to-intermediate signal

  • Inflammation, edema, or tumor infiltration: bright hyperintensity

T1 GRE Fat-Sat Pre-Contrast:

  • Normal body: homogeneous intermediate signal

  • Lesions: hypointense masses (adenocarcinoma, cystic lesions) compared to parenchyma

T1 GRE Fat-Sat Post-Gadolinium:

  • Normal body: intense, homogeneous early enhancement (reflecting rich vascular supply)

  • Tumors: remain hypoenhancing focal areas compared to enhancing pancreas

  • Pancreatitis: patchy, heterogeneous enhancement; necrotic regions remain non-enhancing

  • Cysts: non-enhancing, with enhancing capsule if inflamed

CT Appearance

Non-Contrast CT:

  • Normal body: soft tissue density, slightly higher attenuation than muscle

  • Fat planes: outline gland clearly

  • Tumors: appear as hypoattenuating focal or diffuse areas

Post-Contrast CT:

  • Normal body: enhances intensely and homogeneously in arterial and venous phases

  • Tumors: appear hypoattenuating compared to normal enhancing pancreas

  • Pancreatitis: enlarged gland, heterogeneous enhancement, peripancreatic fat stranding, and fluid collections

  • Cysts/abscesses: low-attenuation lesions; infected cysts show rim enhancement
