Cochlear nerve (Cranial nerve VIII)

The cochlear nerve, also known as the auditory nerve, is a crucial component of the vestibulocochlear nerve (cranial nerve VIII). It transmits auditory sensory information from the cochlea of the inner ear to the brainstem, where it is processed into recognizable sounds. This nerve plays an essential role in hearing and is often evaluated in clinical practice for pathologies such as sensorineural hearing loss, vestibular schwannoma, and other inner ear disorders. Imaging, particularly with MRI and CT, is fundamental in assessing its anatomy and pathology.

Function

  • Transmits electrical impulses generated by sound waves in the cochlea to the cochlear nuclei of the brainstem

  • Works in conjunction with the vestibular nerve, which transmits balance information

MRI Appearance

  • T1-Weighted Images:

    • Normally, the cochlear nerve is isointense to adjacent cranial nerves and brainstem structures

    • Not distinctly visualized unless enhanced by pathology or contrast

  • 3D T2-Weighted Images (e.g., CISS, FIESTA):

    • Provides high spatial resolution

    • The cochlear nerve appears as a small, hypointense (dark) linear structure within the internal auditory canal (IAC), typically anterior-inferior in location

    • Clearly separated from the facial, superior, and inferior vestibular nerves

CT Appearance

  • Not directly visualized as a soft tissue structure due to limited soft tissue contrast

  • Indirect assessment possible by evaluating the size and patency of the cochlear aperture (cochlear nerve canal)

  • Narrow or absent cochlear aperture may suggest cochlear nerve deficiency

  • Bony landmarks of the internal auditory canal and cochlear nerve canal can be well delineated
