Common nasal meatus

The common nasal meatus is a crucial anatomical space within the nasal cavity, lying medial to the three main nasal meatuses (superior, middle, and inferior) and adjacent to the nasal septum. This narrow passageway serves as a conduit for air flow, drainage of mucus, and communicates with the paranasal sinuses. Accurate visualization of the common nasal meatus is essential in assessing nasal pathology, sinus disease, and preoperative planning for endoscopic procedures.

Anatomical Description

  • The common nasal meatus is the longitudinal space between the nasal septum and the lateral nasal wall, running the length of the nasal cavity.

  • It is formed by the union of the superior, middle, and inferior meatuses along the medial wall (septum).

  • Functions as a final pathway for drainage from the respective meatuses before air exits through the posterior choanae.

MRI Appearance

T2-Weighted Images

  • The common nasal meatus is seen as a slit-like, hyperintense (bright) space filled with air or sometimes mucus.

  • The surrounding mucosa may appear as a thin hypointense (dark) rim.

  • Adjacent structures, such as the nasal septum and turbinates, are distinguishable by their intermediate or low signal intensity.

T1-Weighted Images

  • The air within the common nasal meatus is hypointense (black).

  • The mucosa and surrounding soft tissues show intermediate signal intensity.

  • Any pathological soft tissue (e.g., polyps, tumors) may show intermediate-to-high signal, depending on content.

STIR (Short Tau Inversion Recovery)

  • The air remains hypointense (black) within the common nasal meatus.

  • Edematous or inflamed mucosa is highlighted by increased (hyperintense) signal.

  • Useful for detecting subtle mucosal thickening or edema adjacent to the common nasal meatus.

CT Appearance

  • The common nasal meatus appears as a narrow, well-aerated space medial to the turbinates and lateral to the nasal septum.

  • On bone window settings, the bony boundaries (nasal septum, lateral nasal wall, and turbinates) are clearly delineated.

  • Soft tissue window shows the air-filled space as black, with the adjacent mucosa as a thin gray line.
