Deep femoral vein (profunda femoris vein)

The profunda femoris vein, also called the deep femoral vein, is the major venous channel of the thigh, accompanying the profunda femoris artery. It serves as a key tributary of the femoral vein and plays a crucial role in draining venous blood from the deep structures of the thigh. The vein has important clinical significance because of its association with deep vein thrombosis (DVT) and as a landmark in vascular imaging and surgery.

Origin, Course, and Termination

  • Origin: Formed by the confluence of venae comitantes of the profunda femoris artery within the thigh

  • Course:

    • Accompanies the profunda femoris artery, lying medial and posterior to it

    • Receives numerous muscular and perforating tributaries along its course

    • Runs proximally toward the femoral vein

  • Termination:

    • Drains into the femoral vein approximately 8 cm below the inguinal ligament, posterior to the femoral artery

Tributaries

  • Perforating veins of the thigh (from hamstring and adductor compartments)

  • Medial and lateral circumflex femoral veins

  • Muscular veins from quadriceps, adductors, and hamstrings

Relations

  • Anteriorly: Profunda femoris artery and femoral artery branches

  • Posteriorly: Adductor magnus muscle

  • Medially: Femoral vein (into which it drains)

  • Laterally: Vastus medialis and vastus lateralis muscles

Function

  • Primary drainage of the deep muscles of the thigh (adductors, hamstrings, quadriceps)

  • Acts as a major tributary of the femoral vein, channeling blood from thigh musculature toward the external iliac vein

  • Provides collateral venous pathways in cases of superficial femoral vein occlusion

Clinical Significance

  • Deep vein thrombosis (DVT): The profunda femoris vein is a common site of thrombus formation, which may propagate into the femoral vein

  • Vascular surgery relevance: Important for bypass grafts, venous catheterization, and as a collateral venous route

  • Imaging landmark: Critical in Doppler ultrasound, CT venography, and MR venography of the thigh and pelvis

  • Tumor/infection spread: May be invaded or compressed by thigh sarcomas or abscesses

MRI Appearance

T1-weighted images:

  • Lumen normally shows flow void (black signal due to moving blood)

  • Thrombosed vein shows intermediate-to-high signal depending on clot age

  • Surrounding fat appears bright, delineating the vessel

T2-weighted images:

  • Normal patent lumen: flow void (dark)

  • Acute thrombus: variable, may appear bright hyperintense

  • Chronic thrombus: lower signal, sometimes with wall thickening

STIR (Short Tau Inversion Recovery):

  • Normal patent lumen remains dark

  • Slow or stagnant flow may appear bright, which can mimic thrombus

  • Acute thrombus often hyperintense; chronic thrombus appears darker with wall thickening

Proton Density Fat-Saturated (PD FS):

  • Vessel lumen normally dark from flow void

  • Thrombus appears bright after fat suppression

  • Helps distinguish clot from surrounding fat planes

T1 Fat-Sat Post-Contrast:

  • Normal vein enhances with contrast (bright lumen)

  • Thrombosed segment: non-enhancing intraluminal filling defect with enhancing vessel wall

  • Perivascular inflammation may show soft tissue enhancement

CT Appearance

Non-Contrast CT:

  • Vein appears as a soft tissue tubular structure adjacent to the profunda femoris artery

  • Acute thrombus may appear hyperdense compared to flowing blood

  • Chronic thrombus appears isodense to muscle

Post-Contrast CT (CT Venography):

  • Normal vein shows uniform contrast opacification

  • DVT: seen as a filling defect within enhancing vein

  • Collaterals may be visualized in cases of chronic obstruction

  • Surrounding fat stranding may indicate inflammatory or infectious process
