Distal vastus lateralis tendon

The distal vastus lateralis tendon is the terminal tendon of the vastus lateralis muscle, the largest and most lateral component of the quadriceps femoris group. It plays a crucial role in forming the quadriceps tendon, which envelops the patella and continues as the patellar ligament to insert on the tibial tuberosity. The tendon contributes significantly to the stability of the knee joint and extension of the leg. Its close relationship with adjacent muscles, the patella, and retinacular fibers makes it an important anatomical and radiological structure.

Origin and Insertion

  • Origin (muscle fibers): Vastus lateralis arises from the greater trochanter, intertrochanteric line, and lateral lip of the linea aspera of the femur

  • Course: Distal fibers converge to form a broad tendon that passes anterior to the lateral femoral condyle, blending with the quadriceps tendon

  • Insertion: Inserts into the superior and lateral margins of the patella, contributing to the quadriceps tendon and indirectly to the tibial tuberosity via the patellar ligament

Relations

  • Anteriorly: Subcutaneous tissue, skin, and iliotibial tract (laterally)

  • Posteriorly: Lateral femoral condyle, lateral synovial recess of the knee joint capsule

  • Medially: Quadriceps tendon fibers from rectus femoris and vastus medialis

  • Laterally: Vastus lateralis muscle belly, lateral patellar retinaculum, and iliotibial band

  • Inferiorly: Blends into patella and patellar ligament

Nerve Supply

  • Femoral nerve (L2–L4), via muscular branches to vastus lateralis

Arterial Supply

  • Lateral circumflex femoral artery (descending branch)

  • Superior lateral genicular artery

  • Muscular branches from profunda femoris

Venous Drainage

  • Corresponding veins drain into lateral circumflex femoral vein and profunda femoris vein, then into the femoral vein

Function

  • Extends the leg at the knee joint as part of quadriceps femoris

  • Contributes to lateral stabilization of the patella

  • Helps maintain knee alignment during locomotion

  • Provides tensile strength as part of quadriceps tendon-patellar mechanism

MRI Appearance

T1-weighted images:

  • Tendon appears as a low-signal (dark) linear band

  • Fatty tissue surrounding the tendon shows bright signal, improving contrast

T2-weighted images:

  • Normal tendon remains low signal

  • Pathology (tendinosis, partial tear) may show focal or diffuse bright signal within fibers

STIR (Short Tau Inversion Recovery):

  • Normal tendon is dark

  • Pathology (strain, tear, inflammation) shows bright hyperintensity

  • Peritendinous edema also appears hyperintense

Proton Density Fat-Saturated (PD FS):

  • Normal tendon is dark

  • Tendinopathy, tear, or inflammation appears bright against suppressed fat background

T1 Fat-Sat Post-Contrast:

  • Normal tendon shows minimal or no enhancement

  • Tendinopathy shows linear or diffuse enhancement

  • Partial tears show enhancement along margins; complete tears may enhance at retracted ends

CT Appearance

Non-Contrast CT:

  • Tendon appears as a dense soft tissue band anterior to distal femur and patella

  • Chronic degeneration or calcific tendinopathy may appear as calcified foci within tendon fibers

Post-Contrast CT:

  • Normal tendon shows no significant enhancement

  • Inflammation, neoplastic infiltration, or postsurgical changes may show enhancement around or within tendon

  • Peritendinous fat stranding suggests inflammatory pathology
