Facial vein

The facial vein is a major superficial vein of the face, running obliquely from the medial canthus of the eye to the lower border of the mandible. It plays a crucial role in draining deoxygenated blood from the superficial structures of the face into the internal jugular vein. The facial vein is clinically important due to its anatomical course, connections with deeper venous systems, and its role in potential facial infections spreading to intracranial venous sinuses.

Function

  • Drains blood from the superficial structures of the face (skin, muscles, subcutaneous tissue)

  • Receives blood from the angular vein, external nasal vein, superior and inferior labial veins, and deep facial vein

  • Empties into the internal jugular vein, either directly or via the common facial vein

  • Serves as a potential route for spread of facial or dental infections to the cavernous sinus via deep venous connections

MRI Appearance

  • T1-Weighted Images:

    • The facial vein appears as a tubular structure with low signal intensity (dark) due to flowing blood (flow void)

  • T2-Weighted Images:

    • The facial vein remains low signal (flow void), surrounded by higher signal intensity of soft tissue and fat

  • STIR Sequence:

    • The facial vein is a low signal (dark) tubular structure; fat suppression highlights the vessel against suppressed background fat

  • Post-Contrast T1 Fat-Saturated Sequence:

    • The facial vein shows strong enhancement if the flow is slow or turbulent; otherwise, rapid blood flow may still cause a flow void

    • Fat saturation improves visualization by suppressing surrounding fat signal, making the enhanced vessel more conspicuous

CT and CTA Appearance

  • CT (Non-contrast):

    • The facial vein is a thin, low-density tubular structure; often difficult to distinguish from surrounding soft tissues without contrast

  • CT Angiography (CTA):

    • The facial vein enhances as a well-defined, linear, hyperdense structure following intravenous contrast administration
