Floor of the maxillary sinus

The floor of the maxillary sinus forms the inferior boundary of the maxillary sinus cavity and is anatomically related to the alveolar process of the maxilla. It lies in close proximity to the roots of the maxillary premolar and molar teeth, with variable thickness depending on age, dentition, and sinus pneumatization.

The floor slopes inferiorly from medial to lateral and often lies below the level of the nasal cavity floor, making the maxillary sinus the lowest of the paranasal sinuses. Its intimate relationship with the dentoalveolar structures explains its importance in dental surgery, sinus augmentation, and maxillofacial imaging.

Location and Structure

  • Position: Inferior aspect of the maxillary sinus cavity

  • Bone composition: Thin cortical bone with underlying cancellous bone

  • Medial relation: Inferior nasal meatus via the maxillary bone

  • Lateral extension: Toward the zygomatic process of the maxilla

  • Thickness: Highly variable; thinner in adults with fully erupted teeth and sinus pneumatization

  • Inferiorly: Roots of maxillary premolars and molars

  • Superiorly: Air-filled cavity of the maxillary sinus

  • Medially: Lateral wall of the nasal cavity

  • Laterally: Zygomatic process of the maxilla

  • Anteriorly: Anterior maxillary wall

  • Posteriorly: Maxillary tuberosity

Attachments and Coverings

  • Mucosal lining: Schneiderian membrane (respiratory epithelium) adherent to the bony floor

  • Periosteum: Thin layer lining the osseous surface

  • Dental structures: Periodontal ligament spaces may be closely approximated to the sinus floor

MRI Appearance

T1-weighted images:

  • Cortical bone: Very low signal (dark)

  • Cancellous bone marrow: Bright signal due to fatty marrow

  • Sinus cavity: Low signal (air)

  • Mucosal lining: Thin, low-to-intermediate signal layer along the floor

T2-weighted images:

  • Cortical bone: Dark, low signal

  • Bone marrow: Bright, slightly less intense than on T1

  • Sinus air: Signal void (black)

  • Mucosa: Thin intermediate signal outlining the floor

STIR:

  • Bone marrow: Intermediate-to-dark signal under normal conditions

  • Cortical bone: Persistently low signal

  • Mucosal lining: Thin intermediate signal

  • Excellent sequence for uniform suppression of marrow fat

T1 Fat-Saturated Post-Contrast:

  • Bone cortex: No enhancement

  • Bone marrow: Minimal to mild homogeneous enhancement

  • Mucosa: Thin linear enhancement along the sinus floor

  • Surrounding soft tissues: Mild uniform enhancement consistent with normal vascularity

CT Appearance

Non-Contrast CT:

  • Cortical bone: High attenuation, sharply defined inferior sinus wall

  • Cancellous bone: Lower attenuation than cortex

  • Sinus cavity: Air density (black) above the floor

  • Dental root impressions: Smooth elevations or scalloping of the floor contour

  • Best modality for assessing bony thickness and anatomic variations

Post-Contrast CT (standard):

  • Bone: No intrinsic enhancement

  • Mucosal lining: Thin, smooth enhancement

  • Adjacent soft tissues: Uniform enhancement without distortion of normal anatomy

X-Ray Appearance

Occipitomental (Waters) view:

  • Sinus cavity: Radiolucent (dark) area

  • Floor of maxillary sinus: Thin radiopaque curved line forming the inferior boundary
