Folium of Vermis

The Folium of Vermis is a narrow, leaf-like portion of the cerebellar vermis situated in the superior aspect of the posterior cerebellum. It is part of the lobule VII of the vermis, which is located between the culmen anteriorly and the tuber posteriorly. This region is considered to be involved in higher-order coordination and cognitive functions. On the superior cerebellar surface, the folium of vermis is continuous laterally with the folium (folium cerebelli) of the cerebellar hemispheres.

Anatomy

  • Lies in the superior vermis, posterior to the culmen and anterior to the tuber.

  • Forms the central portion of lobule VII A of the vermis.

  • Laterally, it continues with the folium of each cerebellar hemisphere.

  • Appears as a thin, elongated, leaf-like gyrus on the cerebellar surface.

  • Borders:

    • Anterior: Culmen (vermis lobule VI)

    • Posterior: Tuber (vermis lobule VII B)

Arterial Supply

  • Mainly supplied by branches of the Superior Cerebellar Artery (SCA).

  • May receive minor supply from the Anterior Inferior Cerebellar Artery (AICA) depending on anatomical variation.

  • Small cortical branches penetrate the folium and its adjacent white matter.

Venous Drainage

  • Venous drainage is primarily through the superior cerebellar veins, which drain into the straight sinus and transverse sinus.

  • The hemispheric veins adjacent to the folium also contribute to the drainage of this region.

Function

  • Plays a role in the coordination of voluntary movements, especially those requiring fine motor control.

  • Associated with higher-order cognitive functions, including aspects of attention and executive function.

  • May be involved in balance and postural adjustments due to its connections within the cerebellar circuitry.

MRI Appearance

  • On T1-weighted MRI: Appears as a hypointense (dark) band surrounded by hyperintense (brighter) white matter.

  • On T2-weighted MRI: The folium of vermis shows intermediate signal intensity, with clear demarcation from the surrounding white matter.

  • High-resolution MRI may show the leaf-like, foliated structure.

  • Best visualized on sagittal and coronal images through the midline cerebellum.

CT Appearance

  • Appears as a thin, slightly denser (more hyperattenuating) band relative to the surrounding cerebellar white matter, but differences are subtle.

  • Sulci between folia may be accentuated by cerebrospinal fluid (CSF), which appears hypodense.

  • The folium of vermis itself may not be distinctly differentiated on standard CT due to limited soft tissue contrast.
