Gluteal tuberosity

The gluteal tuberosity is a roughened vertical ridge located on the posterior surface of the femur, extending from the base of the greater trochanter downward to merge with the lateral lip of the linea aspera. It serves as the principal insertion site for the gluteus maximus muscle, playing a major role in hip extension strength and posterior thigh contour.

The tuberosity is strong, dense cortical bone reinforced to withstand the powerful pull of the gluteus maximus and stresses transmitted during locomotion, climbing, and lifting.

Location and Structure

  • Situated on the upper posterior femoral shaft, inferior to the greater trochanter

  • Marks the lateral boundary of the proximal femur’s muscular attachment zones

Relations

  • Anteriorly: Femoral shaft cortex

  • Posteriorly: Gluteus maximus muscle

  • Medially: Pectineal line and medial posterior femoral surface

  • Laterally: Vastus lateralis origin zone

  • Superiorly: Greater trochanter

  • Inferiorly: Linea aspera

Attachments

  • Gluteus maximus: Inserts onto the gluteal tuberosity and iliotibial tract

  • Vastus lateralis: Posterior fibers originate partly from the ridge

  • Intermuscular septa: Provide fibrous anchorage along the posterior femur

Function

  • Provides strong bony anchorage for hip extension through gluteus maximus

  • Assists in lateral hip stabilization during gait

  • Contributes to force transmission from pelvis to femur

  • Defines posterior thigh contour and muscular architecture

MRI Appearance

T1-weighted images:

  • Cortex: Very low signal intensity (black)

  • Medullary bone: Bright, due to fatty marrow in adults

  • Gluteus maximus insertion: Low-signal tendinous fibers attaching to the ridge

  • Femoral shaft margins: Sharp, with surrounding muscle of intermediate signal

T2-weighted images:

  • Cortex: Low signal

  • Marrow: Bright, though slightly less intense than T1

  • Muscle: Intermediate-to-low signal

  • Clear contrast between bone cortex, marrow, and gluteal muscle attachment

STIR:

  • Cortex: Dark

  • Marrow: Intermediate-to-dark

  • Surrounding muscle: Intermediate

  • Fat suppression provides clear distinction of marrow and soft tissue planes

T1 Fat-Saturated Post-Contrast

  • Normal cortex: No enhancement

  • Marrow: Mild homogeneous enhancement

  • Gluteus maximus insertion: Smooth, thin enhancement of the myotendinous junction

  • Periosteum and muscular margins: Mild uniform enhancement without irregularity

CT Appearance

Non-Contrast CT:

  • Cortex: High attenuation, thick, well-defined ridge

  • Marrow cavity: Lower-density fatty marrow centrally

  • Gluteal tuberosity: Seen as a linear roughened elevation on posterior femur

  • Surrounding soft tissue: Normal density muscle planes of gluteus maximus and vastus lateralis
