Gracile fasciculus

The gracile fasciculus is an ascending white-matter tract of the dorsal (posterior) columns of the spinal cord. It conveys fine touch, vibration, and conscious proprioception from the lower trunk and lower limbs to the brainstem. It is a key component of the dorsal column–medial lemniscal pathway.

The gracile fasciculus has a predictable midline position and orderly somatotopic organization, making it an important landmark in spinal cord anatomy and neuroimaging.

Location

  • Located in the posterior (dorsal) funiculus of the spinal cord

  • Occupies the medial portion of the dorsal column

  • Present at all spinal cord levels

  • Lies medial to the cuneate fasciculus (above T6)

  • Ascends ipsilaterally to the caudal medulla

  • Terminates in the gracile nucleus of the medulla oblongata

Anatomical course

  • Spinal cord:

    • Composed of central processes of primary sensory neurons from dorsal root ganglia

    • Ascends without synapsing in the spinal cord

  • Medulla oblongata:

    • Fibers synapse in the gracile nucleus

    • Second-order neurons form internal arcuate fibers

  • Beyond medulla:

    • Internal arcuate fibers decussate to form the medial lemniscus

Relations

Posteriorly:

  • Posterior median sulcus

Anteriorly:

  • Posterior gray horn (via dorsal column interface)

Laterally:

  • Cuneate fasciculus (above T6)

Medially:

  • Posterior median septum

Structures conveyed

  • Fine (discriminative) touch from lower limb and lower trunk

  • Vibration sense

  • Conscious proprioception

Function

  • Sensory discrimination: Enables perception of texture, shape, and precise localization

  • Position sense: Conveys joint position and movement from lower extremities

  • Postural control: Supports balance and coordinated movement

  • Cortical sensory integration: Provides high-resolution sensory input to the somatosensory cortex

MRI appearance (normal)

T1-weighted images:

  • Gracile fasciculus: Intermediate signal consistent with compact white matter

T2-weighted images:

  • Normal: Low-to-intermediate signal relative to gray matter

FLAIR:

  • Normal: Typically inconspicuous due to tightly packed fibers

Diffusion-weighted imaging (DWI):

  • Normal: No diffusion restriction

Post-contrast T1-weighted images:

  • Normal: No enhancement

CT appearance (normal)

Non-contrast CT:

  • Gracile fasciculus: Not individually visualized

Post-contrast CT:

  • Normal: No focal enhancement
