Ilioischial line

The ilioischial line is a radiographic landmark seen on an AP pelvis X-ray.

  • It represents the inner (medial) cortex of the posterior column of the acetabulum—formed by the ilium and ischium.

  • On an AP pelvis view, it appears as a smooth curved line running from the pelvic brim down toward the ischium.

Why it matters (clinical use):

  • It helps assess the posterior column and medial acetabular wall.

  • Disruption, displacement, or irregularity of the ilioischial line can suggest acetabular fractures (especially posterior column involvement) or protrusio acetabuli (medial migration of the femoral head/acetabulum).
