Lateral wall of maxillary sinus

The lateral wall of the maxillary sinus forms the outer boundary of the maxillary sinus and separates the sinus cavity from the facial soft tissues and infratemporal region. It is composed primarily of the zygomatic process of the maxilla and contributes to the facial contour and structural integrity of the midface.

This wall is of particular importance in maxillofacial surgery, dental procedures, trauma assessment, and radiologic evaluation of sinus disease and facial fractures.

Paranasal sinus radiographs (Waters’ view):

  • Lateral wall: Smooth curvilinear bony outline of the maxillary sinus

  • Cortical margins: Thin but well-defined

  • Symmetry: Usually symmetric bilaterally

  • Soft tissue: Overlying facial soft-tissue shadow visible

CT appearance

Non-contrast CT:

  • Lateral wall: Thin hyperdense cortical bone forming the sinus boundary

  • Sinus cavity: Air-filled and hypodense relative to bone

  • Zygomatic buttress: Appears as a thicker bony region laterally

  • Continuity: Wall integrity clearly assessed

Post-contrast CT:

  • Bony wall: No enhancement

  • Mucosa: Thin mucosal lining may enhance mildly

Volume-rendered technique (VRT):

  • Lateral maxillary contour: Smooth convex facial surface

  • Zygomatic buttress: Prominent structural pillar

  • Surface anatomy: Clear depiction of maxillary sinus boundaries

  • Utility: Useful for trauma assessment and surgical planning

MRI appearance

T1-weighted images:

  • Cortical bone: Low signal intensity

  • Sinus cavity: Low signal (air-filled)

  • Marrow in adjacent maxilla: Intermediate to high signal

T2-weighted images:

  • Bone cortex: Low signal

  • Sinus air: Signal void

  • Mucosa: Thin low-signal lining
