Medial sural cutaneous nerve

The medial sural cutaneous nerve is a sensory branch of the tibial nerve in the popliteal fossa. It plays a major role in forming the sural nerve by joining with the sural communicating branch (from the common fibular nerve). This nerve provides cutaneous sensation to the posterior and lateral leg and lateral aspect of the foot. It is an important structure in peripheral nerve imaging, surgical graft harvesting, and evaluation of sensory neuropathies.

Origin, Course, Branches, and Insertion

  • Origin:

    • Arises from the tibial nerve in the upper part of the popliteal fossa

  • Course:

    • Descends vertically between the two heads of the gastrocnemius muscle

    • Pierces the deep fascia of the leg to become subcutaneous in the middle third of the calf

    • Travels downward along the posterior calf in close relation to the small saphenous vein

    • Joins the sural communicating branch (from common fibular nerve) to form the sural nerve

  • Branches:

    • Cutaneous branches supplying skin over the posterior calf

    • Communicating branch to unite with sural communicating nerve

  • Insertion (termination):

    • Merges with the sural communicating branch to form the sural nerve, which continues to supply the posterolateral leg, ankle, and lateral foot

Relations

  • Superiorly: Originates from the tibial nerve in the popliteal fossa

  • Anteriorly: Related to popliteal vessels and posterior capsule of knee

  • Posteriorly: Covered by skin and superficial fascia of the calf

  • Laterally: In relation to the lateral head of gastrocnemius and small saphenous vein

  • Medially: Medial head of gastrocnemius and posterior tibial neurovascular bundle proximally

Function

  • Provides cutaneous sensory innervation to the skin of the posterior calf

  • Contributes to the sural nerve, which supplies sensation to the posterolateral leg, ankle, and lateral border of the foot

Clinical Significance

  • Commonly used for nerve graft harvesting in peripheral nerve repair surgeries

  • May be affected by entrapment, trauma, or iatrogenic injury during calf or Achilles tendon surgery

  • Sural neuropathy may cause sensory loss, paresthesia, or burning pain in its distribution

  • Useful landmark in peripheral nerve conduction studies and ultrasound-guided procedures

MRI Appearance

T1-weighted images:

  • Appears as a thin low-to-intermediate signal linear structure within bright subcutaneous and fascial fat planes

T2-weighted images:

  • Nerve shows intermediate to mildly hyperintense signal compared to muscle

  • Pathology (neuritis, injury) appears as brighter hyperintensity

STIR (Short Tau Inversion Recovery):

  • Normal nerve shows low signal

  • Inflamed or edematous nerve shows bright hyperintensity

Proton Density Fat-Saturated (PD FS):

  • Nerve shows low-to-intermediate signal

  • Pathology (injury, inflammation) demonstrates localized bright hyperintensity

T1 Fat-Sat Post-Contrast:

  • Normal nerve shows minimal or no enhancement

  • Pathologic nerve (neuritis, infiltration, neuroma) shows focal or diffuse enhancement

3D T2 SPACE / CISS:

  • Nerve shows intermediate to mildly hyperintense signal compared to muscle

  • Surrounded by bright fat/soft tissue planes, allowing excellent contrast

  • Useful for mapping continuity with tibial nerve proximally and sural nerve distally

CT Appearance

Non-Contrast CT:

  • Nerve not directly visualized; appears as thin soft tissue structure within posterior calf fat planes

  • Small saphenous vein helps as a landmark for its location

Post-Contrast CT:

  • Nerve itself does not enhance

  • Pathologic changes (nerve sheath tumor, inflammatory infiltration) may appear as enhancing nodules or masses along its course
