Nasal crest of maxilla

The nasal crest of the maxilla is a thin, sharp bony ridge located on the medial surface of the maxillary bone, forming part of the floor of the nasal cavity. It provides an important articulation surface for the vomer and contributes to the structural support of the nasal septum.

This structure is a key anatomical landmark in skull base anatomy, otorhinolaryngology, maxillofacial surgery, and imaging of the nasal cavity and septum.

Synonyms

  • Maxillary nasal crest

  • Crest for vomer (of maxilla)

Location

  • Located on the medial surface of the maxilla

  • Runs along the midline of the hard palate

  • Forms the inferior part of the nasal septal framework

  • Inferior to the nasal cavity

  • Superior to the oral cavity (hard palate)

  • Anterior to the palatine bone contribution

Anatomical components

  • Paired nasal crests of the maxillae

  • Midline articulation surface for the vomer

  • Anterior continuation of the palatine process of the maxilla

  • Part of the bony nasal septum framework

Relations

Superiorly:

  • Nasal cavity

  • Nasal septum

Inferiorly:

  • Hard palate

  • Oral cavity

Medially:

  • Vomer

  • Perpendicular plate of the ethmoid (superiorly)

Laterally:

  • Maxillary alveolar process

Posteriorly:

  • Horizontal plate of the palatine bone

Anteriorly:

  • Anterior nasal spine region

Structures related to the nasal crest

  • Vomer

  • Perpendicular plate of the ethmoid (indirect superior relation)

  • Nasal septal cartilage (anteriorly)

  • Nasal cavity floor

Function

  • Septal support: Provides bony attachment and stability for the vomer

  • Nasal cavity integrity: Contributes to separation of the nasal cavities

  • Palatal structure: Participates in formation of the hard palate

  • Midline facial support: Maintains alignment of nasal septum

X-ray appearance

Skull and paranasal sinus radiographs:

  • Nasal crest: Thin midline bony density along the floor of the nasal cavity

  • Continuity: Appears as a linear ridge rather than a separate bone

  • Best seen on: Lateral skull and specialized nasal views

CT appearance

Non-contrast CT:

  • Nasal crest: Sharp midline bony ridge arising from the maxilla

  • Cortical bone: Hyperdense with well-defined margins

  • Relationship: Clearly seen articulating with the vomer

Post-contrast CT:

  • Bone: No enhancement

  • Adjacent soft tissues: Normal mucosal enhancement patterns

3D CT VRT appearance

Volume-rendered technique (VRT):

  • Nasal crest: Visible as a narrow midline ridge on the inferior nasal septum

  • Maxillary contribution: Continuous with palatine process

  • Utility: Useful for preoperative planning and assessment of septal alignment

MRI appearance

T1-weighted images:

  • Nasal crest: Low signal intensity cortical bone

  • Adjacent marrow: Minimal marrow signal due to thin bony structure

  • Nasal septum: Intermediate signal cartilage anteriorly

T2-weighted images:

  • Bone: Low signal

  • Nasal cavity mucosa: High signal outlining the crest
