Posterior fornix of cervix

The posterior fornix is the deepest recess of the vaginal vault, located posterior to the cervix where the vagina reflects onto the uterine cervix. It is the widest and deepest part of the vaginal fornices due to the posterior cervix projecting anteriorly into the vaginal canal. The posterior fornix lies in close proximity to the rectouterine pouch (pouch of Douglas) and is an important anatomical pathway for fluid accumulation, sampling procedures, and gynecologic surgical access.

It provides a compliant space during intercourse, supports cervical mobility, and forms part of the mechanical interface between the uterus, vagina, and posterior pelvic compartment.

Location and Structure

  • Located posterior to the cervical os, forming the posterior extension of the vaginal vault

  • Deepest of the vaginal fornices (posterior > lateral > anterior)

  • Lies directly anterior to the rectouterine pouch

  • Walls composed of stratified squamous epithelium, connective tissue, and smooth muscle continuous with vaginal mucosa and cervix

Relations

  • Anteriorly: Cervix (posterior aspect)

  • Posteriorly: Rectouterine pouch (pouch of Douglas), peritoneum, rectum

  • Superiorly: Uterine isthmus and lower uterine segment

  • Inferiorly: Posterior upper vaginal wall

  • Laterally: Uterosacral ligaments and paracervical connective tissue

Attachments

  • Continuous with vaginal mucosa and cervical epithelium

  • Supported by uterosacral ligaments and pelvic connective tissue

  • Forms the superior extension of the posterior vaginal wall

Function

  • Provides space for cervical movement during intercourse and childbirth

  • Allows expansion of the upper vagina and distributes pressure during coitus

  • Acts as an anatomic interface between cervix and rectouterine peritoneal cavity

  • Clinical access point for culdocentesis and pelvic fluid sampling

Clinical Significance

  • Fluid accumulation: blood, pus, or ascites often collect in rectouterine pouch visible through posterior fornix

  • Site for culdocentesis and transvaginal access to pouch of Douglas

  • Deep fornix tenderness may indicate pelvic inflammatory disease or endometriosis

  • Posterior fornix involvement in prolapse, fistulas, or tumors may alter vaginal contour

  • Important landmark during hysterectomy and vaginal surgical procedures

MRI Appearance

T1-weighted images:

  • Posterior fornix mucosa: intermediate signal similar to other vaginal mucosa

  • Cervical stroma: lower signal than fornix mucosa

  • Peritoneal reflection above fornix: thin low-signal line

  • Vaginal lumen (if fluid present): varies from low to high depending on protein or blood content

T2-weighted images:

  • Mucosa: bright, due to high water content in vaginal epithelium

  • Cervical stroma: low-to-intermediate signal

  • Rectouterine pouch: clearly visible when fluid is present (bright T2 signal)

  • Posterior fornix walls: smooth curvilinear structure with intermediate T2 signal

STIR:

  • Posterior fornix mucosa: intermediate-to-dark depending on fat suppression

  • Any fluid superior to fornix (pouch of Douglas): bright

  • Enhanced contrast between mucosa, surrounding fat, and cervical stroma

T1 Fat-Saturated Post-Contrast:

  • Posterior fornix mucosa: uniform mild enhancement

  • Cervical margins: enhance more than fornix mucosa

  • Peritoneal reflection: thin enhancing line

  • Vaginal wall vascularity becomes more conspicuous

CT Appearance

Non-Contrast CT:

  • Fornix appears as a soft-tissue density curving posterior to the cervix

  • Vaginal wall: homogeneous soft-tissue attenuation

  • Rectouterine pouch: may contain low-density fluid

  • Surrounding fat planes (presacral, pararectal) clearly visible

Post-Contrast CT:

  • Vaginal and fornix mucosa: mild to moderate enhancement

  • Cervix enhances more prominently than fornix

  • Rectouterine peritoneal reflections become sharper

  • Helps delineate posterior fornix thickness and adjacent structures
