Posterior nasal spine

The posterior nasal spine (PNS) is a sharp, midline bony projection located at the posterior margin of the hard palate. It is formed by the horizontal plates of the palatine bones and represents the posterior limit of the bony palate, separating the nasal cavity above from the oral cavity below.

The posterior nasal spine serves as an important anatomical and radiological landmark, particularly in craniofacial imaging, orthodontics, cephalometry, and skull base assessment. It provides attachment to soft palate musculature and plays a role in defining the posterior boundary of the nasal floor.

Location and Structure

  • Position: Midline, posterior end of the hard palate

  • Bone composition: Formed by fusion of the right and left horizontal plates of the palatine bones

  • Shape: Sharp, pointed or slightly blunt bony projection

  • Orientation: Projects posteriorly into the nasopharyngeal space

  • Development: Ossifies as part of the palatine bones during craniofacial development

Relations

  • Superiorly: Floor of the nasal cavity and posterior nasal septum

  • Inferiorly: Oral cavity and soft palate

  • Anteriorly: Hard palate (palatine processes of maxilla and palatine bones)

  • Posteriorly: Soft palate and nasopharynx

  • Laterally: Posterior margins of the horizontal plates of the palatine bones

Attachments

  • Muscular attachments:

    • Musculus uvulae originates near the posterior nasal spine

  • Fibrous attachments:

    • Soft palate aponeurosis attaches to the posterior margin of the hard palate

  • Functional role: Acts as a fixed anchor point for posterior soft palate structures

Function

  • Defines the posterior limit of the hard palate

  • Separates the nasal cavity from the oral cavity posteriorly

  • Provides attachment for soft palate musculature

  • Serves as a key cephalometric landmark in orthodontics and maxillofacial imaging

  • Important reference point in skull base and nasopharyngeal anatomy

MRI Appearance

T1-weighted images:

  • Cortical bone: Very low signal (dark)

  • Marrow (minimal): May show faint bright signal if cancellous marrow is present

  • Surrounding soft tissues:

    • Soft palate muscles: intermediate signal

    • Air in nasal cavity: signal void (black)

  • Posterior nasal spine appears as a sharp low-signal bony projection at the posterior hard palate

T2-weighted images:

  • Bone cortex: Low signal

  • Marrow: Bright if present, limited in extent

  • Soft palate and nasopharyngeal mucosa: Intermediate-to-bright signal

  • Posterior nasal spine remains dark and sharply defined against brighter soft tissues

STIR:

  • Bone: Intermediate-to-dark signal

  • Adjacent soft tissues: Intermediate signal

  • Clearly outlines the posterior nasal spine due to high soft-tissue contrast

T1 Fat-Saturated Post-Contrast:

  • Bone: No enhancement

  • Soft palate and mucosa: Mild homogeneous enhancement

  • Posterior nasal spine remains a non-enhancing low-signal structure, clearly demarcated from enhancing soft tissues

CT Appearance

Non-Contrast CT:

  • Bone: High-attenuation, sharply defined cortical structure

  • Posterior nasal spine seen as a midline pointed bony projection at the posterior hard palate

  • Excellent visualization of its shape, size, and symmetry

  • Surrounding air in nasal cavity provides strong natural contrast

Post-Contrast CT (standard):

  • Bone: No enhancement

  • Adjacent soft tissues: Mild enhancement of soft palate and nasopharyngeal mucosa

  • Posterior nasal spine remains clearly visible as a dense bony landmark

X-Ray Appearance

  • Best visualized on lateral skull radiographs and lateral cephalograms

  • Appears as a sharp radiopaque point at the posterior margin of the hard palate

  • Serves as a standard cephalometric reference point (PNS)

  • Used to assess palatal length, maxillary position, and craniofacial relationships
