Prebiventral fissure

The prebiventral fissure is a relatively minor but distinct anatomical fissure found on the inferior (ventral) surface of the cerebellum. It is located anterior to the biventral lobule (also known as the paramedian lobule) and separates the anterior lobe from the posterior lobe within the cerebellar hemisphere. Although not as prominent as major cerebellar fissures like the primary or horizontal fissure, the prebiventral fissure serves as an important landmark in neuroanatomy and helps demarcate cerebellar lobules during both surgical approaches and radiological assessment.

Arterial Supply

  • Primarily supplied by branches of the Posterior Inferior Cerebellar Artery (PICA)

  • Additional minor supply may arise from branches of the Anterior Inferior Cerebellar Artery (AICA)

Venous Drainage

  • Drains into the inferior cerebellar veins

  • Ultimately empties into the straight sinus and transverse sinus via the cerebellar venous system

Function

  • The fissure itself does not have a direct function but serves as an anatomical landmark

  • The regions it separates are involved in motor coordination, postural control, and fine motor adjustment as part of the cerebellar hemisphere functions

MRI Appearance

  • Appears as a thin, hypointense line (dark) on T1 and T2-weighted images due to the CSF within the fissure

  • Seen best on sagittal and axial sections as a groove anterior to the biventral (paramedian) lobule

  • Can help delineate the boundaries of cerebellar lobules and is often more apparent in high-resolution scans

CT Appearance

  • Typically seen as a linear hypodense (dark) cleft separating adjacent cerebellar lobules

  • Less distinct than on MRI, but may be appreciated in thin-section imaging or with contrast

  • Useful for orientation in cerebellar anatomy, particularly in cases of mass effect, hemorrhage, or congenital anomalies
