Right paracolic gutter

The right paracolic gutter is a vertically oriented peritoneal recess located on the right side of the abdominal cavity. It lies between the lateral aspect of the ascending colon medially and the right abdominal wall laterally. It is one of the four main peritoneal gutters (right, left, and the two paramesenteric spaces), acting as a conduit for peritoneal fluid, infection, and metastatic spread.

The right paracolic gutter communicates superiorly with the hepatorenal recess (Morison’s pouch) and the subphrenic space, and inferiorly with the pelvic cavity. Unlike the left paracolic gutter, which is partially blocked superiorly by the phrenicocolic ligament, the right gutter provides an unobstructed pathway for fluid movement between the pelvis and subhepatic spaces.

Clinically, the right paracolic gutter is important in the spread of infections, intraperitoneal hemorrhage, ascites, and peritoneal metastases. It is a key space evaluated in trauma (FAST/CT), oncology staging, and peritonitis.

Function

  • Provides a dependent pathway for the movement of peritoneal fluid, pus, bile, blood, or tumor cells

  • Functions as a surgical landmark in abdominal exploration and peritoneal lavage

Boundaries

  • Medial: Lateral wall of ascending colon

  • Lateral: Right lateral abdominal wall

  • Superior: Communicates with hepatorenal recess and subphrenic space

  • Inferior: Opens freely into the pelvis

MRI Appearance

T1-weighted images:

  • Peritoneal space normally not visualized unless containing fluid

  • Free fluid appears low signal intensity relative to fat

  • Surrounding peritoneal fat provides high contrast to fluid collections

T2-weighted images:

  • Free fluid appears bright hyperintense in the gutter

  • Allows clear depiction of ascites, pus, or cystic collections

  • Useful in oncology to detect small-volume peritoneal spread

STIR:

  • Fat suppression enhances detection of fluid and edema in the paracolic space

  • Highlights inflammatory or exudative collections as hyperintense

T1 Fat-Saturated (Pre-contrast):

  • Free fluid appears low to intermediate signal against suppressed fat background

  • Inflammatory exudates may appear slightly higher in signal

T1 Fat-Saturated Post-Contrast (Gadolinium):

  • Enhances peritoneal lining and margins of collections

  • Detects peritoneal carcinomatosis, abscess walls, and enhancing nodules within the gutter

MRI Non-Contrast 3D Imaging:

  • Provides 3D mapping of peritoneal gutters

  • Useful in surgical planning and detecting pathways of fluid spread

CT Appearance

CT Pre-Contrast:

  • Right paracolic gutter appears as a fat-filled space lateral to the ascending colon

  • Free fluid, blood, or pus may appear as higher attenuation compared to fat

CT Post-Contrast:

  • Enhances peritoneal margins, inflamed tissues, and masses

  • Free fluid is well visualized in trauma and infection

  • Detects peritoneal carcinomatosis, abscesses, or post-surgical collections
