Right paraspinal line

he right paraspinal line is a normal vertical radiographic interface seen on frontal chest radiographs. It represents the interface between the air-filled lung and the soft tissues adjacent to the right side of the thoracic vertebral bodies, including paraspinal muscles and mediastinal soft tissue.

It is an important mediastinal line used in systematic chest X-ray interpretation to assess symmetry and detect subtle paraspinal or posterior mediastinal abnormalities.

Location

  • Seen along the right lateral margin of the thoracic spine

  • Extends vertically from the upper thoracic levels

  • Typically visible from approximately T1 to T10

  • Located in the posterior mediastinum

  • Medial to the right lung field

Anatomical basis

  • Right lung air column forming the lateral border

  • Paraspinal soft tissues, including:

    • Paraspinal muscles

    • Connective tissue and fat

  • Posterior mediastinal pleura

  • Right lateral surfaces of thoracic vertebral bodies

Relations

Medially:

  • Thoracic vertebral bodies

Laterally:

  • Right lung

Anteriorly:

  • Posterior mediastinum

Posteriorly:

  • Paraspinal muscles

Superiorly:

  • Upper thoracic spine

Inferiorly:

  • Lower thoracic spine

X-ray appearance

Chest radiograph (PA view):

  • Appearance: Thin, smooth, vertical soft-tissue line paralleling the right side of the thoracic spine

  • Contour: Gently undulating following vertebral contours

  • Thickness: Usually narrow and uniform

  • Visibility: More consistently seen on the right than the left due to anatomical asymmetry

  • Extent: Best visualized in the mid-to-lower thoracic region

  • Dependence on technique: Best seen on well-centered, adequately penetrated radiographs
