Soft palate

The soft palate is a mobile, fibromuscular structure forming the posterior continuation of the hard palate, separating the nasopharynx from the oropharynx. It hangs obliquely downward and backward from the posterior edge of the hard palate and ends inferiorly as the uvula.

Unlike the hard palate, the soft palate contains no bone and is composed of muscles, aponeurosis, mucous glands, and lymphoid tissue. Its mobility is essential for speech articulation, swallowing, and airway protection, allowing dynamic closure of the nasopharynx during phonation and deglutition.

Location and Structure

  • Position: Posterior to the hard palate, forming the roof of the oropharynx and the floor of the nasopharynx

  • Components:

    • Palatine aponeurosis: Fibrous framework

    • Muscles: Tensor veli palatini, levator veli palatini, palatoglossus, palatopharyngeus, musculus uvulae

    • Mucosa: Stratified squamous epithelium inferiorly and respiratory epithelium superiorly

  • Free margin: Inferior border ends in the uvula

  • Surface differentiation:

    • Oral surface: Faces oropharynx

    • Nasal surface: Faces nasopharynx

Relations

  • Anteriorly: Posterior edge of hard palate

  • Posteriorly: Oropharynx and nasopharyngeal isthmus

  • Superiorly: Nasopharynx

  • Inferiorly: Oropharynx and tongue base

  • Laterally: Palatoglossal and palatopharyngeal arches (tonsillar pillars)

Attachments

  • Anterior attachment: Hard palate via palatine aponeurosis

  • Inferior extension: Uvula

  • Lateral continuity: Blends with palatoglossus and palatopharyngeus muscles forming faucial pillars

  • Muscular suspension: Anchored to skull base via tensor and levator veli palatini

Function

  • Velopharyngeal closure: Prevents nasal regurgitation during swallowing

  • Speech: Essential for articulation and resonance

  • Swallowing: Elevates to direct food bolus into oropharynx

  • Airway regulation: Adjusts nasopharyngeal airflow during respiration

Clinical Significance

  • Key structure in speech and swallowing evaluation

  • Important landmark in sleep-related airway assessment

  • Evaluated in cleft palate surgery, velopharyngeal insufficiency, and ENT imaging

  • Normal imaging appearance is essential to differentiate from disease

MRI Appearance

T1-weighted images:

  • Appears as a well-defined soft-tissue structure posterior to the hard palate

  • Muscle components: Intermediate signal intensity

  • Palatine aponeurosis: Low signal linear structure

  • Fat planes: Bright signal surrounding soft palate margins

T2-weighted images:

  • Muscle: Intermediate-to-slightly low signal compared to surrounding fat

  • Mucous glands: Mildly hyperintense foci within soft palate

  • Nasopharyngeal air: Very low signal providing contrast

STIR:

  • Soft palate muscles: Intermediate-to-dark signal

  • Fat suppression: Highlights muscular outline against suppressed fat

  • Useful for delineating normal soft tissue anatomy

T1 Fat-Saturated Post-Contrast:

  • Normal enhancement: Mild, homogeneous enhancement of muscular and mucosal components

  • Palatine aponeurosis: Minimal enhancement

  • Vascularity appears symmetric and uniform

CT Appearance

Non-Contrast CT:

  • Soft palate appears as a soft-tissue density structure posterior to hard palate

  • Clearly separates nasopharyngeal and oropharyngeal air columns

  • Muscular bulk and uvula are well visualized

  • Fat planes around palate appear hypodense

Post-Contrast CT:

  • Normal enhancement: Mild, uniform enhancement of soft palate musculature and mucosa

  • Symmetric appearance without focal asymmetry

  • Useful for evaluating soft palate contour and airway relationships

X-ray Appearance

Lateral Neck X-ray:

  • Soft palate seen as a soft-tissue shadow extending posteriorly from the hard palate

  • Forms the anterior boundary of the nasopharyngeal air column

  • Uvula may be visible as a small inferior projection

  • Position varies with phonation and respiration

  • Best appreciated on true lateral projection with adequate neck extension
