Soleal line

The soleal line is an oblique ridge on the posterior surface of the tibia that serves as an important anatomical landmark and muscular attachment site. It marks the origin of the soleus muscle and separates muscular compartments on the posterior aspect of the leg.

It is a key reference point in musculoskeletal anatomy, orthopedic surgery, and cross-sectional imaging of the leg.

Location

  • Situated on the posterior surface of the tibial shaft

  • Extends obliquely from the lateral condyle downward and medially

  • Located inferior to the tibial plateau

  • Above the nutrient foramen of the tibia

  • Separates the upper and lower posterior compartments of the leg

Anatomical components

  • Osseous ridge on the posterior tibial cortex

  • Muscular attachment zone:

    • Soleus muscle

  • Related attachments:

    • Popliteus muscle attaches above the soleal line

    • Tibialis posterior and flexor digitorum longus arise below the line

Superiorly:

  • Popliteal surface of the tibia

  • Popliteus muscle attachment

Inferiorly:

  • Posterior surface of tibial shaft

  • Deep posterior compartment muscles

Medially:

  • Posterior border of the tibia

Laterally:

  • Interosseous border of the tibia

Anteriorly:

  • Tibial shaft and marrow cavity

Posteriorly:

  • Soleus muscle

X-ray appearance

Plain radiographs (AP and lateral leg views):

  • Soleal line: Subtle linear ridge or faint oblique line on posterior tibia

  • Visibility: Best appreciated on lateral views

  • Cortical outline: Smooth and continuous

CT appearance

Non-contrast CT:

  • Soleal line: Clearly defined oblique bony ridge on posterior tibial cortex

  • Cortical margins: Intact and sharply outlined

  • Trabecular pattern: Normal cancellous bone

  • Spatial assessment: Excellent for demonstrating its orientation and extent

MRI appearance

T1-weighted images:

  • Cortical bone: Low signal intensity

  • Marrow cavity: High signal

  • Soleal line: Appears as a low-signal ridge projecting into surrounding muscle planes

T2-weighted images:

  • Cortex: Low signal

  • Adjacent muscles: Intermediate signal

  • Soleal line: Seen as a hypointense linear contour against muscle

STIR:

  • Fat suppression: Uniform marrow fat suppression

  • Muscle planes: Well visualized around the line

  • Soleal line: Maintains low signal

Proton density fat-saturated (PD FS):

  • Excellent contrast between cortical bone and adjacent soft tissues

  • Soleus muscle attachment: Clearly delineated

  • Posterior compartment anatomy: Well defined
