Spinothalamic tract

The spinothalamic tract is a major ascending sensory pathway of the central nervous system responsible for transmitting pain, temperature, and crude touch from the body to the thalamus. It is a key component of the anterolateral system and maintains a relatively consistent position as it ascends through the spinal cord and brainstem.

Due to its long course, predictable anatomy, and compact organization, the spinothalamic tract is an important landmark in neuroanatomy and brainstem imaging.

Location

  • Begins in the spinal cord after synapse in the dorsal horn

  • Decussates near the level of entry through the anterior white commissure

  • Ascends in the anterolateral funiculus of the spinal cord

  • Continues through the brainstem (medulla, pons, and midbrain)

  • Located lateral to the medial lemniscus in the brainstem

  • Terminates in the ventral posterolateral (VPL) nucleus of the thalamus

Anatomical course

  • Spinal cord:

    • Second-order neurons cross within 1–2 spinal segments

    • Ascend contralaterally in the anterolateral column

  • Medulla:

    • Lies lateral to the inferior olivary nucleus

  • Pons:

    • Positioned in the lateral pontine tegmentum

  • Midbrain:

    • Lies lateral to the medial lemniscus and ventral to the tectum

  • Thalamus:

    • Fibers synapse in the VPL nucleus

Relations

Anteriorly:

  • Reticular formation

Posteriorly:

  • Spinocerebellar tracts

Medially:

  • Medial lemniscus

Laterally:

  • Trigeminal sensory pathways

  • Middle cerebellar peduncle (pons level)

Structures conveyed

  • Pain sensation

  • Temperature sensation

  • Crude (non-discriminative) touch

Function

  • Pain transmission: Conveys nociceptive signals from skin and deep tissues

  • Thermal perception: Carries temperature information to higher centers

  • Protective sensation: Enables rapid awareness of harmful stimuli

  • Sensory integration: Works alongside dorsal column pathways for complete somatosensory perception

MRI appearance (normal)

T1-weighted images:

  • Spinothalamic tract: Intermediate signal consistent with compact white matter

T2-weighted images:

  • Normal tract: Low-to-intermediate signal similar to adjacent white-matter tracts

FLAIR:

  • Normal: Typically inconspicuous due to compact fiber composition

Diffusion-weighted imaging (DWI):

  • Normal: No diffusion restriction

  • Tract orientation: Linear configuration along brainstem axis

Post-contrast T1-weighted images:

  • Normal: No enhancement

CT appearance (normal)

Non-contrast CT:

  • Spinothalamic tract: Not individually visualized
