Transverse pontine vein

The transverse pontine vein is a small but significant venous channel located on the anterior surface of the pons, running transversely to drain venous blood from the pons into the basal vein of Rosenthal or other pontine venous networks. It forms part of the brainstem’s superficial venous system and communicates with adjacent veins, including the anterior pontomesencephalic vein and bridging veins connecting to the cerebellopontine venous plexus.

The transverse pontine vein is important in maintaining normal venous drainage of the pons and plays a role in surgical and interventional planning, as well as in the evaluation of pontine venous infarcts, arteriovenous malformations, or tumors compressing brainstem veins.

Function

  • Drains venous blood from the anterior and lateral surfaces of the pons

  • Connects the pontine venous network to the basal vein of Rosenthal

  • Maintains venous outflow and pressure equilibrium in the brainstem

  • Plays a role in cerebrovascular homeostasis and collateral circulation

MRI Appearance

T1-weighted images:

  • The transverse pontine vein appears as a small, linear hypointense structure along the anterior pons

  • Signal intensity is similar to other veins; flow voids may be seen on standard sequences

T2-weighted images:

  • The vein appears hypointense, while surrounding cerebrospinal fluid (CSF) is hyperintense

  • Vessel walls are not distinct unless enlarged or thrombosed

STIR (Short Tau Inversion Recovery):

  • The transverse pontine vein generally remains low signal

  • Useful to detect adjacent edema or pontine pathology

T1 Post-Contrast (Gadolinium-enhanced):

  • The vein enhances as a thin linear structure, clearly delineating its course across the pons

  • Enhancement helps identify venous anatomy, thrombosis, or compressive lesions

CT Appearance:

  • On non-contrast CT, the transverse pontine vein is not directly visualized

  • On CT venography, the vein appears as a small enhancing channel on the anterior pons
